Responsible for overall management of a bereavement program. Responsibilities include the planning, implementation, maintainance and provision of services necessary to patients, identified caregivers, hospice staff, and the community at large. Hospice bereavement services extend for the time period of the patient’s admission to hospice services until at least thirteen months after the death of the patient.
Provides grief and bereavement support to patients and families consistent with needs of the patient. Serves as a member of the Interdisciplinary Team to develop the plan of care. Acts as liaison between community and hospice program. Adheres to system and department compliance policies, and any and all applicable laws and regulations. Performs other duties as assigned.
Bachelor's Degree-In social work, psychology, counseling, related field or pastoral education.
Documented evidence of training and experience in grief therapy and bereavement counseling helpful.Recent (within the past 5 years) experience in acute, long-term care or home care/hospice, with at least one in hospice preferred. Experience and professional training in grief and bereavement care preferred. Understanding of physical, psychosocial and spiritual needs of terminally ill patients and their caregivers.
Computer skills to include Microsoft Work and Excel and the ability to work independently
Must be self-motivated, good listener, and have excellent interpersonal and organizational skills
Must possess and demonstrate excellent verbal and written communication skills and demonstrate competence in organizing and presenting educational programs. Belief in the hospice concept.
