Medical Social Worker

This is a per diem position located in Greenfield, MA. Position is day shift and is a part of the weekend call rotation (1-2 weekends per month).

Provide social work services to hospital patients and their families. Services are provided upon referral by nursing staff/physicians, community contacts, or by patients or their families. Services address those areas of psycho-social functioning that adversely affect or are affected by the patient's medical condition, coping skills, and/or discharge status. Interventions include direct counseling of patients and families regarding emotional, financial, environmental and safety issues as well as appropriate referrals and coordination with other community agencies. Social work services are provided for the medical/surgical units including adults and children, ICU, obstetrics/nursery, oncology and the emergency department.

Job Responsibilities: 

1) Provide initial and ongoing psycho-social assessments of patients referred. Demonstrate knowledge about human behavior and theory; and about the impact of disease on individuals and families as it relates to their age and stage of development. Discuss patients' physical progress with physicians, nursing staff and other disciplines via multi-disciplinary rounds and individual contact with professional staff.

2) Provide financial resource information to patients and families. Make referrals, as needed, to home health/home care agencies, nursing homes, substance abuse programs and other social service/community agencies or programs.

3) Review and discuss psycho-social and health care needs with appropriate staff members of the transfer agency. Provide assessment of abuse/neglect for children, disabled adults, elders and others. File reports with appropriate agencies as needed. Provide ongoing follow-up with patients, families and agencies as needed.

4) Work with hospital attorney regarding legal issues such as guardianship or commitment of those with alcohol or other substance abuse. Provide assessment of medical inpatients for possible discharge to a mental health unit. Make referrals as needed to mental health units or to outpatient mental health services as appropriate. Obtain insurance approvals for these services (inpatient or outpatient) when required. Provide DSM multiaxial assessment for such patients as needed.

5) Use CIS as a means of multidisciplinary communication. Complete all appropriate documentation, forms and applications in a timely manner. Demonstrate clear written and verbal communication. Provide consultation to Baystate Franklin Medical Center staff and physicians regarding psycho-social issues. Participate in staff meetings and present cases as part of peer review.

6) Work closely with Case Management Department to review cases for acute care needs. Participate on committees as assigned. Complete forms for departmental statistics.

7) Participate in quality and performance improvement activities as assigned. Attend in-service and education programs to maintain and enhance knowledge of current theory and practice and to maintain licensure.

8) Provide in-service education to department staff on relevant educational programs attended. Provide on-call beeper coverage on weekends as assigned. Adhere to system and department compliance policies, and any applicable laws and regulations.

9) Observe all health and safety requirements. This description covers the essential functions of the position. Incumbent is expected to perform other similar and related duties as assigned. Management has the right to change or add to the job responsibilities at any time.

10) Demonstrate and model principles of service excellence in support of the patient experience and SPIRIT Program.

Required Work Experience: 

1) One or more years of experience in a medical setting

Preferred Work Experience: 

1) Experience in an acute care setting preferred or 2-3 years experience in a related medical setting
