Case Manager position at the Child Partial Hospital Program (CPHP)- Per Diem Position
Case manager (CM) position at the Child PHP (CPHP), a replacement for a staff person leaving for another job. The CM position is a primary role at the CPHP, a main driver of clinical treatment and care management of our acute psychiatric child/adolescent patients. CMs are responsible for intake evaluations, intensive treatment planning, direct clinical care (including individual and group therapy), family meetings, crisis management, and discharge planning (including coordination with community providers). Additionally, CMs assist in the safety management of the milieu. Without this position we would not be able to meet our regulatory clinical requirement standards or our budgeted census goals.
Case Manager position at the Child Partial Hospital Program (CPHP)
Under the direction of the Program Manager and Clinical Supervisor, the Case Manager II in the Child Partial Hospital Program is responsible for performing a wide range of duties involving patient evaluation, case management, treatment, discharge planning and documentation. The Case Manager II is responsible for gathering a psychosocial evaluation through interviews and record review and completing an assessment with case formulation and treatment plan. The Case Manager facilitates psychoeducational and psychotherapeutic groups. Position includes close collaboration with the treatment team, including a Psychiatrist, Clinical Nurse Specialist, Cinical Supervisor, Case Managers and Psychiatric Nurse. Case Manager II must have excellent written, keyboarding and verbal communication skills. The position must represent and support our mission to provide quality, compassionate care to members of our community while operating in collaboration with other clinical departments, emphasizing the important interaction between behavioral health, general health and wellness and contributing appropriately to the overall financial objective of Baystate Health.

- Master’s Degree
- LICSW Licensed Indepentent Clinical Social Worker or equivilant
- Minimum 3 years of experience
